Please complete the form accurately, as you will manually add your membership.

1. Submit your payment by cash or Bank Transfer and ensure to keep your receipt for reference. All payments from Jamaica are to be made to Foamagic Limited - Scotia Bank Business Current a/c #301359 (Falmouth Branch). Shamar Wellington is the ONLY Authorized agent for Foamagic in Jamaica.
2. Memberships are: Emerald JMD$2,000 - Sapphire JMD$4,000 - Diamond JMD$7,000.
3. To complete the membership set-up, you will be emailed a discount code to set "purchase" your membership. Your profile will automatically upgrade to the membership site, and you will be able to track membership from your regular login.
4. This is done by adding the selected membership to your cart and entering the discount code at checkout. There will be no charge to do this since your payment will be verified prior to receiving the code.

Please note that if you purchase the membership directly from the website without the discount code, the cost will be higher, and FREE months will not be included.
